Question: What is the “ISNT rule” in glaucoma?

Answer: The ISNT rule is an easy way to remember how the optic nerve is supposed to look in a normal eye. Normally the neuro-retinal rim is thickest Inferiorly and thinnest Temporally.

With glaucoma, however, you begin to see vertical thinning, with atrophy along the inferior and superior rims. Thus, when optic nerves don’t follow the ISNT rule, they may have had glaucomatous damage.

that`s anatomical,….. then functional would be your AVF and then your ^ IOP……. DX ~ Glau

WHAT IF INFERIOR AND SUPERIOR RIM’S ARE THINNED OUT BUT BOTH EYES HAVE SIMILAR CD RATIO(0.5) . VERTICAL HORIZONTAL DIFFRENCE OF 0.1 ONLY. WITH NO INCREASE IN IOP AND PATIENT HAVING NO MODIFIABLE RISK FACTORS SUCH AS BP, DM, SMOKING, NORMAL LIPID PROFILE MALE 40YR. SHOULD WE STILL GO FOR VF
